In the Item Information box - the section "I am a(an)*" referst to the who is donating the item...if it is a business donating this item, please select business. If it is a person donating the item, please select individual. 

If your basket/package contains 5 items, please submit a form for each item - by clicking on the "donate another item" button at the bottom after submission.

If you took a package/basket name from Ashley, use that name. If you are creating your own basket - then make up your own name. Please add your best, most fun, intersting, inspiring basket/package descrition - or if you are drawing a blank...leave it blank!
